ACT / CSI Caseworker
Department: ACT / CSI
- High School Diploma or GED or 5 years experience working with individuals with mental illness.
- Must have valid driver’s license and auto insurance.
- Provide mental health rehabilitation interventions and supports to individuals and groups with a mental illness, which is determined necessary to achieve rehabilitative, resilience and recovery goals primarily in a person’s own environment, at least 60%
- Coordinate and assist with the identification of individual strengths, resources, preferences and choices.
- Assist with the identification of existing natural supports for development of a natural support team.
- Assist with the development of crisis management plans.
- Assist with the identification of risk factors related to relapse and development of relapse prevention plans and strategies.
- Support and promotion of client self-advocacy and participation in decision-making, treatment and treatment planning.
- Assist the client to build a natural support team for treatment and recovery.
- Skill building in order to assist the client in the development functional, interpersonal, family, coping and community living skills that are negatively impacted by the client’s mental illness.
- Act as a liaison consult with community agencies and families to maintain coordination in the treatment process.
Breastfeeding Peer Counselor
Department: WIC
- Has breastfed at least one baby within the last 5 years (does not have to be currently breastfeeding).
- Previous or current WIC participation or, other women from the community with breastfeeding experience and an enthusiasm for breastfeeding may be recruited, hired and trained to be peer counselors.
- A woman of the community, with similar characteristics (i.e. age, race, cultural ethnicity, education and socioeconomic level) of WIC participants
- Has a telephone and is willing to make phone calls from home.
- Has reliable transportation.
- Attends breastfeeding training classes to become a peer counselor.
- Receives a caseload of WIC mothers and makes routine periodic contacts with all mothers assigned.
- Gives basic breastfeeding information and support to new mothers.
- Counsels WIC pregnant and breastfeeding mothers by phone, home visits, and /or hospital visits at scheduled intervals determined by the local WIC program.
- Is available outside the WIC clinic and the usual 8-5 working schedule to new mothers who are having breastfeeding problems.
- Individuals must demonstrate great cultural and socioeconomic sensitivity to populations served
First Episode Psychosis (FEP) Integrated Recovery Therapist (IRT)
- Master’s degree in human services
- Valid driver’s license, reliable transportation, and the ability to provide services in community settings.
- Coordinate the overall delivery of FIRST IL services using a person-centered, recovery-oriented, and shared decision-making approach.
- Conduct intake assessments for individuals referred from hospitals, crisis units, and community providers.
- Lead weekly FIRST IL multidisciplinary team meetings to coordinate treatment, review progress, and address participant needs.
- Provide evidence-based Individual Resiliency Training (IRT) using the approved FIRST IL curriculum and Cognitive Behavioral Therapy for Psychosis (CBT-p) principles.
- Teach skills that help individuals better understand and manage symptoms, increase resilience, and improve daily functioning.
- Help individuals develop healthy coping strategies, problem-solving skills, stress management techniques, and independent living skills.
- Engage family members and natural supports identified by the individual in treatment whenever appropriate and authorized.
- Provide Family Psychoeducation using the approved FIRST IL Family Education and Support curriculum.
- Connect families with community resources, including support groups and educational organizations.
- Coordinate services with hospitals, primary care providers, behavioral health providers, schools, employers, and other community partners.
- Monitor participant progress and modify interventions as needed to support recovery goals.
- Participate in clinical supervision, continuing education, and required FIRST IL training.
Home Health RN
Department: Home Health
- Valid Illinois RN license
- One or more years of community health nursing experience.
- High energy level and empathetic ability in communication and human relations.
- Must have valid driver’s license, availability of transportation and must have auto insurance.
- Is informed and involved with initial evaluation, start of frequency of visits and method of payment on all home health clients.
- Regularly re-evaluates the individual’s nursing needs.
- Is responsible for maintaining Home Health Program standards as outlined in the Rules and Regulations for Home Health agencies.
- May perform services in the homes of individuals within designated counties in accordance with the medical treatment plan for each individual patient made by the attending physician in conjunction with the EHD staff.
- Provides the following services: Evaluates the level of function, follows the plan of treatment, prepares clinical and progress notes, advises and consults with the family and other agency personnel and participates in in-service programs.
- Maintain an up-to-date clinical record for each patient receiving care.
- Provide progress notes to the patient’s physician about patients under care when the patient’s condition change or there are deviations from the plan of care or at least every sixty days.
- Individuals must demonstrate great cultural and socioeconomic sensitivity to populations served.
Public Health Nurse - RN
- Valid Illinois RN license
- Reports directly to the Maternal Child Health Coordinator.
- Perform WIC assessment, nutrition education, counseling, and other requirements as outlined in the state and local WIC policy and procedure manual.
- Administer WIC benefits and tailor client food packages as appropriate.
- Perform nursing assessment and other requirements as outlined in the state and local Better-Birth-Outcomes- Comprehensive (BBO-C) Policy and Procedure Manual.
- Will conduct client appointments in office, by telephone, and/or offsite as needed.
- Demonstrate a positive and supporting atmosphere for breastfeeding, and will obtain breastfeeding lactation certification when available.
- Administer and prepare childhood and adult immunizations per CDC and IDPH guidelines.
- Schedule and perform DNA testing as requested by commercial laboratories.
- Attend and/or participate in program outreach activities and events.
- Assist and document client referrals as needed to appropriate providers and/or resources.
Substance Use Disorder Counselor
- Degree in a human service field.
- Must have either an Illinois Certification (ICB) or be able to obtain certification within two years of hire. Other licensures relevant to the position will be considered.
- Assume case management/therapist responsibilities for a caseload of clients with alcohol and drug related problems.
- Provide follow-up services to clients discharged from treatment facilities.
- Conduct substance use disorder groups.
- Maintain complete documentation in client records as required by Illinois Administrative rules.
