CCSO Intensive Care Coordinator
Department: CCSO
- Bachelor’s degree in Health and Human related field OR
- High School Diploma with 5 years verifiable clinical experience in mental health or human services OR
- Family partnership professional with current certification from the Illinois Alcohol and Other Drug Abuse Professional Certification Association.
- Provide intensive care coordination to youth in Tier 2, maintaining an average caseload of 16 youth not to exceed 18 youth on a caseload at one time.
- Coordinate and facilitate Wraparound team meetings and assign responsibilities/activities to members on each wraparound team.
- Maintain comprehensive Plan of Care and update after each team meeting or as needed.
- Assist with the identification, recruitment, and engagement of existing natural supports for development of a natural support team.
- Assist with the development of crisis safety plans.
- Act as a liaison and consult with community providers and families to maintain coordination in the treatment team process.
- Educate families, providers and the community about the wraparound process.
- Participate in all HFS Intensive Care Coordination and High-Fidelity Wraparound trainings and quarterly boosters, as well as in complete yearly certification to administer IM-CANS lifespan assessment and other screenings with clients as required.
Community Health Worker - Integrated Care for Adults (InCA)
Department: INCA
- High School Diploma or equivalent with any combination of 3 years health/social services experience.
- Experience in engaging community members.
- Excellent computer skills, task oriented.
- Preference will be given to candidate with experience working with clients with psychiatric or substance use issues and complicated medical needs.
- Must have valid driver’s license, transportation and evidence of insurability.
- Help clients in utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible.
- Coach clients in effective management of their chronic health conditions including mental health and substance use disorder.
- Assist clients in developing and understanding care plans.
- Document activities, care plans, and results in an effective manner while strictly adhering to the policies and procedures in place.
- Work collaboratively and effectively within a team.
- Make contact with disengaged attributed clients via telephone, mail, and face to face interactions in the home/community.
- Uploads appropriate information to EHR and data collection systems within specified time frames.
- Contact all clients after behavioral health hospitalizations to complete a Transition of Care form and uploads into HealthEC.
- Attends weekly staffing to update team on progress.
Public Health Nurse - RN
- Valid Illinois RN license
- Reports directly to the Maternal Child Health Coordinator.
- Perform WIC assessment, nutrition education, counseling, and other requirements as outlined in the state and local WIC policy and procedure manual.
- Administer WIC benefits and tailor client food packages as appropriate.
- Perform nursing assessment and other requirements as outlined in the state and local Better-Birth-Outcomes- Comprehensive (BBO-C) Policy and Procedure Manual.
- Will conduct client appointments in office, by telephone, and/or offsite as needed.
- Demonstrate a positive and supporting atmosphere for breastfeeding, and will obtain breastfeeding lactation certification when available.
- Administer and prepare childhood and adult immunizations per CDC and IDPH guidelines.
- Schedule and perform DNA testing as requested by commercial laboratories.
- Attend and/or participate in program outreach activities and events.
- Assist and document client referrals as needed to appropriate providers and/or resources.
