Data Analyst
Department: Integrated Care for Kids
- Bachelor’s $41,000 or Masters $48,000 in Data Analytics, Healthcare Management or Policy, Health Education or related field.
- Must have prior experience with evaluation and data collection processes.
- Under the direct supervision of the Village InCK Project Director.
- THIS POSITION IS ON SITE. NO REMOTE WORK.
- Ensure systems/processes are in place for data collection and data management.
- Oversee Village InCK data collection and management for the purpose of program model goals, evaluation, reporting outcomes, maintaining funding and promoting sustainability.
- Developing, reporting and monitoring of AIMS Driver for Village InCK.
- Provide Data reporting for Village InCK Quarterly and Annual Reports.
- Attend and report data outcomes at Partnership council and Project Officer meetings.
- Work with Project Director and leadership at EHD to develop strategies to improve outcomes for Village InCK using data for insight.
- Develop understanding and proficiency at reporting on the Village InCK Alternative Payment Model. Provide APM Village InCK provider participants, MCO’s, State Partners and CMS with quarterly updates on progress of APM metrics in comparison to baseline.
- Work with IT and finance department to process and monitor the Village InCK Alternative Payment Model.

Diversity, Equity and Inclusion Specialist
Department: Integrated Care for Kids
- Requires a bachelor’s or master’s degree in human services or related field (e.g., social work, psychology, education, business or public administration).
- Experience working with a diverse group of community agencies, organizations, or other resources.
- Provide leadership around DEI community programming and education to ensure the delivery of training and resources to core child services that are responsive to the changing needs of marginalized populations.
- Organize or present at events and trainings with topics that are centered around DEI curriculum for various constituencies in the community on behalf of Village InCK and Egyptian Health Department.
- Organize and lead the Village InCK Family Advisory Committee meetings held monthly.
- Represent Village InCK DEI on community committees that champion a broad range of issues.
- Collaborate with community partners and establish relationships with marginalized communities to bridge services, education opportunities and connection within the community.
- Assist in staff development in a manner that promotes not only a respectful workplace culture but also inspires professionalism across diverse constituencies.

LPN
Department: Home Health
- Currently licensed in the State of Illinois as a Licensed Practical Nurse.
- Graduate of Accredited Practical Nursing Program.
- Physical requirements include: Lifting (up to 50 lbs); kneeling and bending.
- Good speaking, hearing and communication skills.
- Ability to deal with the public courteously and tactfully.
- Ability to follow oral and written instructions and to work as a team.
- Must have valid driver’s license and auto insurance.

RN
Department: Home Health
- Registered Nurse in the State of Illinois.
- One or more years of community health nursing experience.
- High energy level and empathetic ability in communication and human relations.
- Must have valid driver’s license, availability of transportation and must have auto insurance.
- Is informed and involved with initial evaluation, start of frequency of visits and method of payment on all home health clients.
- Regularly re-evaluates the individual’s nursing needs.
- Is responsible for maintaining Home Health Program Standards as outlined in the Rules and Regulations for Home Health agencies.
- May perform services in the homes of individuals within designated counties in accordance with the medical treatment plan for each individual patient made by the attending physician in conjunction with the EHD staff.
- Provides the following services: Evaluates the level of function, follows the plan of treatment, prepares clinical and progress notes, advises and consults with the family and other agency personnel and participates in in-service programs.
- Maintain an up-to-date clinical record for each patient receiving care.
- Provide progress notes to the patient’s physician about patients under care when the patient’s conditions change or there are deviations from the plan of care or at least every sixty days.

SASS Counselor
- A bachelor's degree is required in a human service field.
- Will have experience working with high needs families and adolescents at risk of being removed from their home, school or community.
- Will provide crisis intervention assessment services to all referrals for children/youth.
- Will provide intense therapy to children/youth referrals, and their families
- Develop service plans in conjunction with service recipients needs while they are hospitalized, after hospitalization and/or following deflection from inpatient services.

Service Integration Coordinator
Department: Integrated Care for Kids
- Requires a bachelor's degree in human services or related field (e.g. social work, psychology, education, business, nursing, or other healthcare-related degrees).
- Experience working with a diverse group of community agencies, organizations, or other resources.
- Requires knowledge of various community resources, have excellent interpersonal skills, verbal and written communication skills.
- Work within multi system / software platforms for both referral and data collection responsibilities.
- Serve as single point of service coordination contact for InCK designated Medicaid recipients age 0-21 and /or their caregiver(s).
- Provides outreach and coordination contact for core (partner) family and child-serving agencies and InCK beneficiaries and their caregiver.
- Coordinates and processes screenings for InCK beneficiaries.
