Assessment Coordinator
Department: Village InCK
- Bachelor's degree required.
- Preference given to experience in direct service, or coordination and information gathering for patient care.
- Experience preferred working with consumers to schedule and gather information, communication skills, organizational skills, and working with information and data input.
- Will lead the Service Integration Level (SIL) Checklist (information and data collection) of Medicaid kids in the 5-county area for the InCK model project.
- Oversee the collection of data and information to assist in decision making for the level of care necessary for kids screened in 5 county InCK model (Saline, Gallatin, White, Hamilton and Wayne).
- Updating and gathering information for a Universal Care Plan for kids that have higher level of healthcare need within a Population Health Management Platform.
- Become and remain certified with the IM-CANS - a behavioral health assessment tool to determine level of treatment/care.
- Work with InCK staff (Service Integration Coordinators) on referrals, assessments and determination of care for higher needs kids when indicated.
- Work with leadership staff to troubleshoot and problem solve issues.
- Gather information and data input for SIL Checklist and Universal Care Plans.

Assessment Specialist
- This position requires a Bachelor's or Master's Degree in human services, a Registered Nurse with at least one year experience or a Certified Psychiatric Nurse.
- This person will provide comprehensive assessments according to the regulatory agencies requirements.
- Maintains up-to-date client records including support data, billing, etc. on a timely basis.
- Provides case management services, as necessary to link to the appropriate services.
- Demonstrates the ability to provide care coordination services.
- Individuals must demonstrate great cultural and socioeconomic sensitivity to populations served.
Cultural and Linguistic Coordinator
Department: CMHC
- Bachelor’s degree in a human services field.
- A minimum of 3-5 years related experience working with culturally diverse populations.
- Possess the ability to read, analyze and interpret relevant materials.
- Possess the ability to present information and respond to questions from staff and stakeholders.
- Serves as an advocate for minority families within the five county area served.
- Provides technical assistance to partners to insure for cultural competency in their service delivery and works with the evaluation team to address cultural competency issues as they arise.
- Oversees the completion and implementation of the cultural competency plan.
- Coordinate with Social Marketing Coordinator and CMHC Director to insure for cultural competency in all communications.
- Responsible for insuring those components of CMHC are culturally appropriate and that staff are culturally competent.

Intensive Care Coordinator
Department: CCSI
- Bachelor’s degree in Health and Human related field OR High School Diploma with 5 years verifiable clinical experience in mental health or human services OR a family partnership professional with current certification from the Illinois Alcohol and Other
- Provide intensive care coordination to youth in Tier 2, maintaining an average caseload of 25 youth not to exceed 30 youth on a caseload at one time.
- Coordinate and facilitate Wraparound team meetings and assign responsibilities/activities to members on each wraparound team.
- Coordinate and assist with the identification of individual strengths, resources, preferences and choices to create a strengths-based team vision.
- Assist with the identification, recruitment and engagement of existing natural supports for development of a natural support team.
- Assist with the development of crisis safety plans.
- Assist family in drafting a family history to inform treatment and treatment goals.
- Act as a liaison and consult with community providers and families to maintain coordination in the treatment team process.
- Educate families, providers, and the community about the wraparound process.
- Provide transportation in a personal vehicle and agency vehicle as needed.

Mental Health Therapist
- Bachelor's Degree in Human Services, however preference will be given to Master's level or licensed applicants with a LCSW, LCPC or LPC.
- Duties will include counseling services, treatment team meetings, case management and community support services.

Recovery Oriented System of Care (ROSC) Mentor
Department: ROSC
- High School Diploma or equivalent
- Experience working with substance use disorder recovery supports in Region 5
- Preference will be given to someone who has “lived experience” in navigating oneself or a loved one through the substance use disorder system of care.
- Help champion recovery for the region.
- Engage in statewide Recovery Council.
- Help coordinate statewide collaborative ROSC meetings.
- Attend monthly site-specific ROSC council meetings of each grantee.
- Support community activities, provide additional support, build PLE(People with Lived Experience) capacity.
- Provide required documentation and reports.
- Facilitate roundtable discussions/learning collaborative meetings with regional councils and at statewide meetings.
- Offer support, discover barriers if ROSC groups are missing deadlines.
- Regularly meet with community mentor liaisons, TA team, and leadership team.
- Individuals must demonstrate great cultural and socioeconomic sensitivity to populations served.
Service Integration Coordinator
Department: InCA
- Requires a bachelor’s degree in human service or related field (e.g., social work, psychology, education, business, nursing, or other healthcare-related degrees).
- Experience working with a diverse group of community agencies, organizations, or other resources.
- Must be able to work within multi system/software platforms for both referral and data collection responsibilities.
- Requires knowledge of various community resources, excellent interpersonal skills, verbal and written communication skills.
- Serve as the single point of service coordination contact for InCA designated Medicaid recipients age 21 and older through the iHUB.
- Conducts Comprehensive Needs Assessment and Screenings for all adult beneficiary referrals and walk-ins.
- Supports and tracks linkages/referrals to community-based resources.
- Report and advocate for change based on needs indicators.
- Works closely with the direct service team and other key staff to ensure comprehensive care is provided.
- Provides outreach and coordination contact for core partner agencies and InCA beneficiaries.
- Provide referral and linkage information for collaborators and InCA recipients including stratification to medically necessary levels of care using the IM CANS needs assessment as a guide.
- Will prompt, record and ensure care tasks within a Population Health Management system (i.e,follow-ups, wellness visits, screenings, care plans, etc.) are completed by appropriate direct service staff to improve health outcomes of InCA recipients.
- Establish and further develop collaborative working relationships as an informational contact for agencies, service providers, and Medicaid beneficiary families in the counties served.

Substance Use Disorder Counselor
Department: SUDS
- Must have a Bachelor’s or Master’s Degree in a human service field.
- Must have either an Illinois Certification (ICB) or be able to obtain certification within two years of hire. Other licensures relevant to the position will be considered.
- Assume case management/therapist responsibilities for a caseload of clients with alcohol and drug related problems.
- Provide follow-up services to clients discharged from treatment facilities.
- Conduct substance use disorder groups.
- Maintain complete documentation in client records as required by Illinois Administrative rules.
Youth Mental Health Professional
- This position will be at the Eldorado office.
- Will provide specialized mental health treatment services to youth and their families ages 0-21.
- Requires a Bachelor's Degree in human services related field, Master's Degree preferred.
- This candidate will have an understanding and acceptance of System of Care values and principles and have excellent oral and written communications skills.
- Will provide direct services in home, office or community based settings to families and children.
- Have knowledge of treatment strategies in child behavior management.
- The candidate will have the ability to work with families in a supportive manner throughout the diagnostic and treatment process as well as coordinate with providers in other service systems.
- The candidate must demonstrate sensitivity to the cultural and socioeconomic characteristics of families served.
