CCBYS Counselor
Department: CCBYS

CCSO Intensive Care Coordinator
Department: CCSO
- Bachelor’s degree in Health and Human related field OR
- High School Diploma with 5 years verifiable clinical experience in mental health or human services OR
- Family partnership professional with current certification from the Illinois Alcohol and Other Drug Abuse Professional Certification Association.
- Provide intensive care coordination to youth in Tier 2, maintaining an average caseload of 16 youth not to exceed 18 youth on a caseload at one time.
- Coordinate and facilitate Wraparound team meetings and assign responsibilities/activities to members on each wraparound team.
- Maintain comprehensive Plan of Care and update after each team meeting or as needed.
- Assist with the identification, recruitment, and engagement of existing natural supports for development of a natural support team.
- Assist with the development of crisis safety plans.
- Act as a liaison and consult with community providers and families to maintain coordination in the treatment team process.
- Educate families, providers and the community about the wraparound process.
- Participate in all HFS Intensive Care Coordination and High-Fidelity Wraparound trainings and quarterly boosters, as well as in complete yearly certification to administer IM-CANS lifespan assessment and other screenings with clients as required.
Engagement Specialist - Crisis Services
Department: Crisis
- High School Diploma or GED
- Have lived experience that has navigated or been a consumer of a mental health system.
- Night Shift- Sunday & Monday 8:00 p.m. to 9:00 a.m.,Tuesday 8:00 p.m. to 8:00 a.m.
- Provide engagement services with Mental Health Professional during face to face screening and assessments for individuals in need.
- Assist Mental Health Professional with completion of screenings for substance use, social determinants of health and any other assessment screenings to be completed during assessment process for individuals in need.
- Provide peer engagement services.
- Provide transition, linkage and after care resources to referrals.
- Provide crisis deflection services within 24 hours to individuals deflected from a higher level of care.

Home Health LPN
Department: Home Health
- Currently licensed in the State of Illinois as a Licensed Practical Nurse.
- Graduate of Accredited Practical Nursing Program.
- Physical requirements include: Lifting (up to 50 lbs); kneeling and bending.

Home Health RN
Department: Home Health
- Registered Nurse in the State of Illinois
- One or more years of community health nursing experience.
- Must have a valid driver's license, availability of transportation and must have auto insurance.
- Is informed and involved with initial evaluation, start of frequency of visits and method of payment on all home health clients.
- Regularly re-evaluates the individual’s nursing needs.
- Is responsible for maintaining Home Health Program standards as outlined in the Rules and Regulations for Home Health agencies.
- May perform services in the homes of individuals within designated counties in accordance with the medical treatment plan for each individual patient made by the attending physician in conjunction with the EHD staff.
- Provides the following services: Evaluates the level of function, follows the plan of treatment, prepares clinical and progress notes, advises and consults with the family and other agency personnel and participates in in-service programs.
- Maintain an up-to-date clinical record for each patient receiving care.
- Provide progress notes to the patient’s physician about patients under care when the patient’s condition change or there are deviations from the plan of care or at least every sixty days.

Screening, Assessment and Support Services (SASS) Counselor/Therapist - Norris City Annex
Department: SASS
- Masters OR Bachelor’s in Social Work or Human Services field. Preferably with 3 years of full-time job experience working with children and families.
- Will have experience working with high needs families and adolescents at risk of being removed from their home, school or community.
- Provide crisis intervention assessment services to all referrals from CARES portal for children/youth in imminent danger of being placed in a psychiatric facility within the time frames and guidelines of the program, and to determine need for inpatient
- Provide intensive counseling/therapy to children/youth, their families and other collaterals, and develop service plans in conjunction with service recipients while hospitalized, after hospitalization, and/or following deflection from inpatient services
- Complete records and reports correctly on appropriate forms and in a timely manner consistent with the needs as designated in the SASS program plan.
- Provide transportation as needed by service recipients.
- Participate in the delivery of emergency and crisis intervention services to clients as part of the 590 crisis system.
- Promote establishment of and participation in local services, facilities, and activities designed to stabilize the child/youth in his /her placement.
- Establish and maintain working relationship with allied resources including courts, law enforcement, school personnel, other social agencies, etc.
Youth Mental Health Professional - Carmi
- Will provide specialized mental health treatment services to youth and their families ages 0-21.
- This candidate will have an understanding and acceptance of System of Care values and principles and have excellent oral and written communications skills.
- Requires a Bachelor's Degree in human services related field
- This position will be at the Carmi office.
- Will provide direct services in home, office or community based settings to families and children.
- Have knowledge of treatment strategies in child behavior management.
- The candidate will have the ability to work with families in a supportive manner throughout the diagnostic and treatment process as well as coordinate with providers in other service systems.
