Assessment Specialist
- This position requires a Bachelor's or Master's Degree in human services, or a Registered Nurse with at least one year experience or a Certified Psychiatric Nurse
- This position will work out of the Eldorado office.
- This person will provide comprehensive assessments according to the regulatory agencies requirements.
- Maintains up-to-date client records including support data, billing, etc. on a timely basis.

Community Health Worker - Integrated Care for Adults (InCA)
Department: Integrated Care for Adults (InCA)
- High School Diploma or equivalent with any combination of 3 years health/social services experience.
- Experience in engaging community members.
- Excellent computer skills, task oriented.
- Preference will be given to candidate with experience working with clients with psychiatric or substance use issues and complicated medical needs.
- Must have valid driver’s license, transportation and evidence of insurability.
- Help clients in utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible.
- Coach clients in effective management of their chronic health conditions including mental health and substance use disorder.
- Assist clients in developing and understanding care plans.
- Document activities, care plans, and results in an effective manner while strictly adhering to the policies and procedures in place.

First Episode Psychosis (FEP) Integrated Recovery Program (IRT) Therapist
Department: CMHC
- Bachelor’s Degree in a human service field
- Completes intake assessment for FIRST referrals at inpatient psychiatric hospitals, crisis units or in the agency’s office.
- Conducts outreach to increase referrals to FIRST.
- Assists the case manager and other team members as needed to ensure continuity of care and that the client’s needs are being met by FIRST services.
- Engages in regular consultation meetings with the FIRST consultant/trainer.
- Participates in monthly multi-disciplinary First Episode Psychosis learning collaborative.
- Responsible for a small case load of clients for whom individual/group counseling sessions along with case management will be provided.

Home Health LPN
Department: Home Health
- Currently licensed in the State of Illinois as a Licensed Practical Nurse.
- Graduate of Accredited Practical Nursing Program.
- Physical requirements include: Lifting (up to 50 lbs); kneeling and bending.
- Good speaking, hearing and communication skills.
- Ability to deal with the public courteously and tactfully.
- Ability to follow oral and written instructions and to work as a team.
- Must have valid driver’s license and auto insurance.
- Provides services in accordance with agency policies including maternal and child health programs.
- Performs home health visits and other related duties under the supervision of an R.N.
- Follow plan of care initiated by the R.N.
- Assists in coordination of appropriate referrals to other disciplines and to insurance companies.
- Prepares clinical progress notes within 24 hours of a home visit.
- Assess and provides client and family/care giver education and information pertinent to diagnosis/plan of care.
- Uses universal precautions while administering care to clients and is knowledgeable of policies and procedures and protocols regarding infection disease.

Home Health RN
Department: Home Health
- Registered Nurse in the State of Illinois
- One or more years of community health nursing experience.
- Must have a valid driver's license, availability of transportation and must have auto insurance.
- Is informed and involved with initial evaluation, start of frequency of visits and method of payment on all home health clients.
- Regularly re-evaluates the individual’s nursing needs.
- Is responsible for maintaining Home Health Program standards as outlined in the Rules and Regulations for Home Health agencies.
- May perform services in the homes of individuals within designated counties in accordance with the medical treatment plan for each individual patient made by the attending physician in conjunction with the EHD staff.
- Provides the following services: Evaluates the level of function, follows the plan of treatment, prepares clinical and progress notes, advises and consults with the family and other agency personnel and participates in in-service programs.
- Maintain an up-to-date clinical record for each patient receiving care.
- Provide progress notes to the patient’s physician about patients under care when the patient’s condition change or there are deviations from the plan of care or at least every sixty days.

School Based Youth Mental Health Professional
Department: Youth Mental Health
- Bachelor’s degree and/or Master’s degree, clinical license in the State of IL, and/or certifications in evidence-based practices.
- Promote healthy social and emotional development in all children and youth served.
- Provides counseling/therapy services to individuals, groups, families, and classrooms.
- Provide comprehensive assessment, diagnostic crisis screening, referral services, developmental screenings, and direct therapeutic service to clientele.
- Provide consultation and support to caregivers and other providers as well as coordinating services to maximize resources and minimize duplication of services for clientele.
- Prepares client records, reports, data and required statistics in a timely fashion.
Screening, Assessment and Support Services (SASS) Counselor/Therapist - Norris City Annex
Department: SASS
- Masters or Bachelor’s in Social Work or Human Services field. Preferably with 3 years of full-time job experience working with children and families.
- Will have experience working with high needs families and adolescents at risk of being removed from their home, school or community.
- Establish and maintain working relationship with allied resources including courts, law enforcement, school personnel, other social agencies, etc.
- Promote establishment of and participation in local services, facilities, and activities designed to stabilize the child/youth in his /her placement.
- Participate in the delivery of emergency and crisis intervention services to clients as part of the 590 crisis system.
- Attend all staffings as requested by DCFS regarding service recipients.
- Provide transportation as needed by service recipients.
- Complete records and reports correctly on appropriate forms and in a timely manner consistent with the needs as designated in the SASS program plan.
- Provide intensive counseling/therapy to children/youth, their families and other collaterals, and develop service plans in conjunction with service recipients while hospitalized, after hospitalization, and/or following deflection from inpatient services
- Provide crisis intervention assessment services to all referrals from CARES portal for children/youth in imminent danger of being placed in a psychiatric facility within the time frames and guidelines of the program, and to determine need for inpatient
Substance Use Disorder Counselor
Department: SA
- Must have a Bachelor's in a human service field.
- Must have either an Illinois Certification (ICB) or be able to obtain certification within two years of hire. Other licensures relevant to the position will be considered.
- Assume case management/therapist responsibilities for a caseload of clients with alcohol and drug related problems.
- Provide follow-up services to clients discharged from treatment facilities.
- Conduct substance use disorder groups.
- Maintain complete documentation in client records as required by Illinois Administrative rules.
Youth Mental Health Professional
Department: C & A Division
- Requires a Bachelor's Degree in human services related field
- Will provide specialized mental health treatment services to youth and their families ages 0-21.
- This candidate will have an understanding and acceptance of System of Care values and principles and have excellent oral and written communications skills.
- This position will be at the Eldorado office.
- Will provide direct services in home, office or community based settings to families and children.
- Have knowledge of treatment strategies in child behavior management.
- The candidate will have the ability to work with families in a supportive manner throughout the diagnostic and treatment process as well as coordinate with providers in other service systems.
- The candidate must demonstrate sensitivity to the cultural and socioeconomic characteristics of families served.
