Build, Amplify, Support, Empower (BASE) Prevention Specialist

Department: Public Health
- Requires a master’s degree in community health education; OR Requires a bachelor’s degree with courses in community education with areas of concentration in the behavioral and biological sciences, and experiences in community health education;
- Reports directly to the Director of Health Education.
- Supervises the BASE program staff including approving time off, conducting performance evaluations and present discipline actions, if needed.
- Assist in the development and implementation of an arts-based prevention program in Gallatin, Hamilton, Saline, Wayne, and White counties.
- Support staff and/or provide evidence-based youth prevention education in the classroom.

Engagement Specialist - Crisis Services
Department: Crisis
- High School Diploma or GED
- Have lived experience that has navigated or been a consumer of a mental health system.
- Night Shift - Wednesday & Thursday 8:00 p.m. to 9:00 a.m., Friday 8:00 p.m. to 8 a.m.
- Provide engagement services with Mental Health Professional during face to face screening and assessments for individuals in need.
- Assist Mental Health Professional with completion of screenings for substance use, social determinants of health and any other assessment screenings to be completed during assessment process for individuals in need.
- Provide peer engagement services.
- Provide transition, linkage and after care resources to referrals.
- Provide crisis deflection services within 24 hours to individuals deflected from a higher level of care.
- Individuals must demonstrate great cultural and socioeconomic sensitivity to populations served.
- Increase engagement in care by assisting families/individuals in moving through a mental health system from hospitalization and/or deflection, through the agency intake process and transition from crisis to other levels of services.
- Assist individuals/families/caregivers in understanding, seeking and achieving the best possible outcomes for themselves or a family member (including complete recovery) by supporting consumer rights and empowering individuals/families/caregivers
Health Educator
Department: Public Health
- Requires a master’s degree in community health education; OR Requires a bachelor’s degree with courses in community education with areas of concentration in the behavioral and biological sciences, and experiences in community health education
- Plans, organizes, and conducts comprehensive public health education programs focused on Sexual Risk Avoidance Education and positive youth development.
- Advises local public health groups to assist them in developing and improving the health education aspects of their programs.
- Plans and conducts programs designed to promote and stimulate interest in health education; speaks before various community groups; prepares presentations, lecture materials, and reading lists; operates presentation technology.
- Implements components of a variety of health education grant deliverables.

LPN
Department: Home Health
- Currently licensed in the State of Illinois as a Licensed Practical Nurse.
- Graduate of Accredited Practical Nursing Program.

Service Integration Coordinator
Department: Integrated Care for Adults
- Requires a bachelor’s degree in human services or related field (e.g, social work, psychology, education, business, nursing, or other healthcare-related degrees).
- Experience working with a diverse group of community agencies, organizations, or other resources.
- Must be able to work within multi system/software platforms for both referral and data collection responsibilities.
- Requires knowledge of various community resources, excellent interpersonal skills, verbal and written communication skills.
- Serve as the single point of service coordination contact for InCA designated Medicaid recipients age 21 and older through the ihub.
- Conducts Comprehensive Needs Assessment and Screenings for all adult beneficiary referrals and walk-ins.
- Supports and tracks linkages/referrals to community-based resources.
- Report and advocate for change based on needs indicators.
- Works closely with the direct service team and other key staff to ensure comprehensive care is provided.
- Provides outreach and coordination contact for core partner agencies and InCA beneficiaries.
- Provide referral and linkage information for collaborators and InCA recipients including stratification to medically necessary levels of care using the IM CANS needs assessment as a guide.
- Will prompt, record and ensure care tasks within a Population Health Management system (i.e, follow-ups, wellness visits, screenings, and care plans, etc.) are completed by appropriate direct service staff to improve health outcomes of InCA recipients.
- Establish and further develop collaborative working relationships as an informational contact for agencies, service providers, and Medicaid beneficiary families in the counties served.

Wraparound Care Coordinator
Department: CCSO
- Requires Master’s or Bachelor’s degree in Health and human related field
- Provide intensive care coordination to youth in Tier 1, maintaining an average caseload of 10 youth not to exceed 12 youth on a caseload at one time.
- Participate in all HFS intensive care coordination trainings and quarterly boosters, as well as yearly certification to administer IM-CANS lifespan assessment and other screenings with clients as required.
- Responsible for assisting families in identifying natural and professional supports to create a Child and Family Team and facilitating CFTMs every 30 days.
- Act as a liaison and consult with community providers and families to maintain coordination in the treatment team process.
- Educate families, providers and the community about the wraparound process.
- Provide transportation in a personal vehicle and agency vehicle as needed.
