You are invited to participate in the Healthcare Integration Program (HIP). This site provides you with information about the program, and explains the benefits of receiving integrated care to promote health and wellness.
What is the HIP Program?
The HIP Program is part of a federal grant to help improve each person’s health and wellness by coordinating mental health and primary care services. The mission of the HIP Program is to coordinate your mental health and primary healthcare services, provide person-centered healthcare, promote wellness and recovery, and prevent chronic illness.
What are the services offered in the HIP Program?
Services offered by the HIP Program include: Mental Health Services, Primary Care Health Services, Substance Reduction Services, Cooking and Healthy Eating Classes, Weight Management Classes, Smoking Cessation Classes, Diabetes Education Classes, Anger Management Classes, Exercise Activities, Wellness Educational Presentations, and other health and wellness activities.
What are the benefits of the HIP Program?
It is our goal to help you live a healthy life. Each individual who participates in the program will have access to a range of effective health services, support services, and resources to promote wellness, manage illnesses, and improve your overall health.
Who can participate in the HIP Program?
You are eligible to participate in the HIP Program if you are 18 years of age or older AND are receiving mental health or substance abuse services at Egyptian Health Department.
What’s the process for participating?
If you are interested in participating in this program, you can complete a brief referral so that we can contact you. Once we have your referral, you will be contacted for an intake appointment for some important health screenings, which include height, weight, blood pressure, cholesterol, blood sugar, and carbon monoxide in your breath. You’ll also be asked questions to help us improve our program. These screenings will be repeated every 6 months for as long as you are in the program to track your progress, and for each time you participate in the interview, you may be eligible for a $20 gift card.
